Cumulative Update for Windows 10 Version 1511 for x64-based Systems (KB3140768) Failed

There's an update for my Windows 10 (Cumulative Update for Windows 10 Version 1511 for x64-based Systems (KB3140768)). Whenever I restart my pc, it says "Fatal Error C00000D4 applying update operation 540 of 13142 (sqmapi.dll)". How to solve this problem??

Try using sfc /scannow to see if it's a correctable error, if not you might need to manually install 1511 (possibly reinstall windows) using https://www.microsoft.com/en-us/software-download/windows10
