D-SUB out of range 75khz/60hz in games

I cannot play games in full screen mode or my monitor turns black and tells me that it is out of range. How can I fix this?

What is the model of your monitor?

Run (Windows Key + R), type "dxdiag.exe"; on the "Display" tab you should see "Monitor: xxxxxx" under the "Device" section if the driver is installed.

What is the full error?

The whole computer is not affected, i was using a TV (HDMI) but i didn't like it. My GPU does not have a VGA connection so i am now using an adapter from DVI to VGA. This has only occurred recently.
