Disk at 100 percent.

So, I've had my pc with windows 10 for round a year now, and just yesterday it started to get very slow, and i couldnt load into any games or do anything on it. I noticed disk was at 100% Usage and knew something was wrong. I spent all night doing what i had read to do, virus scans, turn off windows search, stuff like that. None of it worked and i started defragmenting its at 22% but i dont know if it will make a diffrence.i havent downloaded any new software of put any new hardware in my pc. Is ot a dead or failing hdd? Specs: AMD A10 6800K, Evga 750ti, 1tb toshiba hdd 10gb of ram.
