Disk is at 100% even though I reinstalled Windows 10

I have had my laptop (HP Envy x360) for about a year. It was slowing down so I tried to figure out what was wrong. Turns out my task manager said told me my disk was almost constantly at 100%. Googling didn't provide a solid answer for why this happens or how to fix it.

Some extra details, it has 8GB of RAM, an i7, and a 930m graphic card. If you need anything else let me know. Thanks for your help.

The "100%" is referring to its current usage, not its capacity. Use http://www.hdtune.com/ to check the health of your HDD.
