[SOLVED] Display crashes/black screen

Currently having an issue where my video signal freezes for a second then crashes. Both monitors go black and lose signal, if I have a video playing sometimes I will be able to hear audio continue playing shortly before being cut off. This happens randomly, watching videos, playing games, or just sitting idle. The only thing I can find in Event Viewer relating to this is the message "Display driver nvlddmkm stopped responding and has successfully recovered." Making me believe it might be a graphics card issue.

My setup:
Windows 10 Pro 64 bit
EVGA GeForce GTX 970
ASUS Rampage IV Extreme
i7-4930k
Corsair Vengeance 16 gig RAM
EVGA 600w power supply
2 monitors - 1 with dual link DVI, the other with VGA

What I've tried to solve it:
reinstall graphics drivers
reinstall windows
take apart hardware, clean it off, and put it back together
memtest came back clean
all temperatures are normal, no fan issues

Any help is appreciated.

You already checked software, time to test the video card in another system with a known good power supply and another card in your system.
