Display goes black while hard drive/computer stays active

Hello,
As of recent I've come across this issue where after some time on the computer my display goes black. While my display goes black the hard drive is still active and I can still interact with the computer.

This often happens when I'm watching youtube videos or when I'm gaming. My display goes black and to get my display back I simply power off and power back on my monitor real quick and everything is back to normal for a short time. This happens once every 5-10 minutes. I want to stress that while the display is black, the computer is not frozen or soft locked.

I'm stumped. Any suggestions?

8192 MB RAM
GeFroce GTX 750 TI

What is the make/model of your display? How do you connect to it (HDMI, VGA, DisplayPort, DVI)?

Are you using any adapters to connect to your display?

Have you tried using different video cables or checked that both ends of your video cable are firmly attached?

My monitor is a Samsung Synchmaster P2450.
I've tried connecting via VGA and HDMI to no success.

Do you have access to another display or HDTV? If so, try using that to isolate the problem.
