DL-DVI to VGA not working

I have a ZOTAC GeForce GTX 750Ti (ZT-70605-10M) with a single DL-DVI port. My monitor is a Samsung SyncMaster 720N with a single VGA port. The monitor and graphics card both work separately and I am able to run my computer using the motherboard graphics. But when I try to connect via the graphics card using the DL-DVI-VGA adaptor that came with the card, the screen does not respond and displays nothing. Is there a hardware problem or do I need to change the output manually?

The adapter should connect directly to the DVI output, not using any DVI cable.
This should give you an analog output that you can use with a VGA cable.

Do you see the boot sequence at all? Is it just Windows that you can't display?
The monitor has maximum resolution 1280x1024. If you are swapping monitors and have Windows set to a higher resolution, it won't work.
If you are just swapping monitors in Windows, drop the resolution to 1280x1024 on the other monitor, shut the PC down, connect the VGA adapter and monitor and then boot the PC.
