Does the Radeon XFX model RX-560P4S need the external power source, if I have 12 v at the PCI Express slot?

Yes, it needs a 6-pin PCIe connector from the power supply. This is not optional, the PCIe express slot cannot provide enough power, therefore, the GPU needs extra power from the power supply.
