Double Revert

After dying and quickly reverting to the last checkpoint, a few times during
play, the game reverted back to the second to the last CP. Seems like a
glitch - like the game can't keep up...

Has this happened to anyone else?

Yes, when there was no way I could survive that checkpoint it went to the
2nd to last.
