Dramatically Slower boot times

Recently( a few months) I have been have painfully slow boot times and by slow I mean it usually takes 20-40min to startup and occasionally I get errors logging in where I enter the correct password and it will try for 5 minutes give me an error, I retype the password and then it works fine. My system used to take about 30sec to a min. The longest part of the boot up is when I sign in and it goes to the desktop. Very often it asks me if I want to end a program or wait for a response. I have noticed that when I get task manager open the disk is almost always 100% when its slow but not every time.

There really is no excuse for my system to be running this slow but I dont know whats causing it. My systems specs are:

AMD FX-8350 Eight Core
EVGA GTX 970 SSC
16GB ADATA 1600Mhz RAM
Thermaltake 750W Smart Series 80 Plus Bronze
ADATA 128GB SSD
Windows 10 Home 64Bit

-Note that the OS is not on the SSD-

There is also a problem with restarting where my mouse, monitors and keyboard will not turn on but the computer and desk mounted mic will light up and work properly.

*I want to avoid a full system reset if possible and only as a last resort.

I have run malwarebytes and various other programs with no violations, I run CCleaner at least twice a month.
 
Its A Seagate Barracuda 2TB.

Any overclocks which I had before have been set to default as of about a month ago ( this problem has been going on longer ) The GPU has a factory OC and I never touched the memory clock settings.

Memtest before had no hiccups. I will try using the manufacturers diagnostic now. I also saved the System Event Log but I'm not sure how I would give it to you.
 
I forgot to mention that I have been putting the computer in Hibernate a lot recently as it takes a really long time to startup and I am wondering if that because I'm not shutting the pc down regularly it could be bad for the overall performance of the startup.
 
It said smart fail the first time then it corrected itself so im going to run a full repair anyway tomorrow when im away as I need the pc currently instead of it being out of order for long periods of time.
