Driver crashes and Blocked Grahpics hardware. Posibly dying AMD GPU?

Hey everyone, I've been having this problem for a few months now, it is most notable on Fallout 4, where I can get the game to run smoothly at 60 FPS on my XFX 270, But after sometimes anywhere from a minute to maybe about 4 or 5 at the longest I've seen. Ending with normally my game suddenly starting to look vary distorted. I have got a lot of images from GRID 2 which is another game that tends to crash, but is much more stable.

Mostly lots of missing and or black textures, polygons stretching. lighting being completely removed from everything besides particles. Rarely but sometimes 'A Long green 45 degree angle strip' Some other games are just fine though, Played the recent overwatch beta without any problems, GTA 5 tends to do the same as above but not as often, mostly just randomly closes without the little window in the corner telling me the game has been blocked. Easy to render games like 'Jet Set Radio' will end up also 'glitching up' after long play time on a single session.

So, With this whole thing everything is vary uncertain.

This is what I have Already tried! Been dealing with this for awhile so this list is probably longer then what I am going to type here.

  • -Update Drivers, currently running on the newest beta of AMD's 200x drivers.
    -Downgrading Drivers, This also didn't help.
    -Cleaning Registry, Did surprisingly help a vary slim bit, as everything doesn't crash as soon, but still does.
    -I haven't overclocked Anything.
    -Upgrading my PSU, I had an old crappy one and upgraded to an EVGA one.
    -Temps are all low. Computer is basically as well ventilated as it can get, I do live in a hot place though.
    -Lowering Graphics quality on basically everything I play. Low, High, Crashes just the same.
    -Checked Ram stability, Its good.
    -Ran a Fur Mark Stability Test, Passed.
    -Ran Hard Drive Tests, Passed.
    -CPU Stability, Passed.
    -Viewing the 'Reliability monitor' window just simply says 'Hardware Error'

So, I am on my final leg right now, The only thing left to do, Is try and replace the GPU, The only other GPU I have access too without buying a new one is my old GPU and that was damage when I removed it from my old Mobo.

WELL Here we are, Maybe you guys might know the problem, Or just tell me that my GPU is dying and that I should get a replacement. I haven't contacted XFX yet about a replacement.

While Writing this I noticed this. Probably nothing to do with the problem. but thought it was a bit funny non then the less.

You say the temps are good when the errors occur, but could you please tell me what the graphics card temps are when the errors occur? HWiNFO is a good monitoring program.

Have you tried upping the graphics card's voltage a bit?
 
Using Corsair link Idle for me is about 42C and 58C is the temp it crashed at (test I did just now.). I haven't tried that yet, I'm assuming you do this in bios options?
(I know corsair link isn't the best but it does show temps.)

I've tested the temps before and they haven't ever been vary hot so i'm not sure whats up.
 
Hey everyone, Still looking for some answers. still no fix for me yet, any ideas?
Tried Boosting my voltage and doesn't seem that I can? Id just like some verification before I return the gpu to the manufacturer.

Update: just tried cleaning and re-mounting my GPU still the same thing, Only other thing I feel like I haven't tried is re-installing windows.
