Dying GPU ?

Hello , i have recently started having problems with my R9 290 vapor-x by sapphire . The drivers started crashing , and the screen would go black , everything would still be on , normally even the GPU itself , but no display (i know the drivers crashed because of the reliability monitor). I have checked my ram and its ok and my PSU is more then enough to hold this 290 (RM 750x by corsair) . I dont overclock and the GPU is 1.5 year old, i have uninstalled the drivers and tried installing them again , but the AMD installer crashed on about 12 %, and i have tried older versions of the driver installer and have used AMD cleanup utility also , i have tried resetting the CMOS battery on the MOBO... i dont know what to do anymore , if u can please do help !

intel core i5 6500

16 gb ddr3 1866mhz ram

R9 290 vapor-x 4gb

ASROCK h110m-DVS/D3

rm 750X Corsair psu

Cooler master hyper TX3 evo cooler/heatsink

1TB WD HDD

Solution
first thing i would do is run display driver uninstaller (DDU) for AMD cards and then after its done reinstall the driver. DDu has fixed more GPU issues for me then any other fix. If DDU doesn't work and an OS re-install (last ditch effort) doesn't work then you'll know the GPU is likely dead or in need of some new thermal paste.
i didnt try it , wait just a minute , i will try fallout 4

no , its like im running on a potato , framerate is shit , the GPU lights up and the monitor is plugged into it, but it doesnt use it....

Btw in device manager it says device is not configured correctly (code 1 )

Solution
ok so..... the driver installer crashes like before , and when i try to do it trough device manager it says windows has encountered a problem installing the driver software for your device ....
 
Alright , i reinstalled windows , everything is working , thank you so much , it kept all my games and files so its not a problem , im just glad the GPU is not dead
