EVGA GTX 970 not working

So I booted up my computer and my screens lasted for a minute before everything died. Now when I power up my computer with the graphics card installed, the fans run at full speed or not at all and I receive no output signal. This card is two years old at this point and nothing significant has changed in the last few months. Thanks for any help!

Test the card in another computer with a known good power supply, if it does the same thing there, card is bad.
