External Hardrive keeps disconnecting and reconnecting when transferring data

Hi. Whenever I'm trying to transfer data from the computer, the hard drive keeps disconnecting then instantly reconnects. Also, when it connects to the computer or when I plug it in, an error message comes up saying there's a problem with the device. Furthermore when trying to scan and repair with chkdsk it fails and says try again. Its a Hitachi 1TB Thanks
