'Extra' fees and surcharges for cell phone plans?

So I'm shopping for my first real* cell phone, and I see plans priced like "$___ plus taxes and other charges/surcharges/fees". I know taxes are dependent on one's nation, state/province, etc., but what are the names of -- and typical dollar (or % of base plan price) costs -- of these fees and "other charges"?

How much extra do you pay above the quoted (base) plan rate? . . . . without counting taxes (that are very location-dependent)

* by 'real' I mean a smartphone with a monthly plan, not a flip phone with a refill-as-I-get-close-to-empty plan (which I have now and really love, but I suspect that few, if any, carriers offer that anymore)
 
In my country, Sweden, the extra fees would include when you are out of reach of your phone providers network, and have to pay extra for using another provider. How much this would be depends on what your provider has agreed to pay the other provider for their customers use of the other providers network. This is also valid when travelling abroad.
