My case is an Antec NX800, which has 2 pre-installed 200mm fans on front, spot for 1 rear and 2 top.

Mine is set up with 2 x 200mm at front, 2 x 120mm at top and 1 x 120mm at rear, have been using it fine temperature of components not too bad. But not sure if I have them set up for good airflow/cooling. How should I have them set up ? as of right now I believe it sucks air in back and blows out front and top

Opinions vary.
A pair of 200mm intakes is great.
As a plus, if all intake is from one place, and is filtered, your parts will stay cleaner.

The rear 120 exhaust serves mainly to direct the airflow over the components.
I would remove the two top fans.
Even if mounted as exhaust(never intake) they divert the airflow from the motherboard, cpu and graphics card. The intake air exits the system without doing anything good.

The top fans are part of my cpu water cooler though

AIO mounting is catch 22.
Do you want best cpu cooling, then mount the radiator as intake.
A lousy option in your situation.
Better to mount the radiator as exhaust.
A bit more info would be helpful.
What is the make/model of your cpu, cooler, and graphics card?
Are you overclocking anything.
What is this pc used for?
