A fatal hardware error has occurred.

Component: AMD Northbridge
Error Source: Machine Check Exception
Error Type: HyperTransport Watchdog Timeout Error
Processor APIC ID: 0

I have just change the PSU so it's running stable now at 11.88V on the 12v rail in full load so that seems to be ok but i still get a lot of crashes..especially when trying to watch a movie or browsing any website

Solution
crashing can easily cause file corruption, did you run an system file checker scan?
Which OS is this?

for win7 or 8 Simply open the Start menu, click All Programs, click Accessories, right-click Command Prompt, and choose Run As Admin¬istrator. Respond to the User Account Control prompt. Then, at the Command Prompt, type sfc /scannow.

The utility will scan your system files and attempt to repair any damage that it finds. Note that it might ask you for the Windows 7 distribution media in order to carry out these repairs.

What motherboard: make, model.
I battled this same thing last spring and summer (2013) and that error message is telling you that the motherboard is dieing. I, too, hoped it was the PSU and replaced same, but to no avail. Rebuilt with an ASUS board and AMD 6300 CPU; haven't had a problem since. Old mother board was a 2 1/2 year old MSI 790XT.
