Fist of Suns and Phage

What is the interaction between Fist of Suns and Phage? Does the caster die
if he plays Phage using Fist of Suns instead of the mana cost printed in the
top right corner?

Nope. Phage doesn't ask "if you didn't play it for the mana cost printed on
the card". It asks "if you didn't play it from your hand". The triggered
ability doesn't care whether you got to pay a reduced cost, had to pay some
additional cost, or used some other cost altogether; all it cares about was
"did you play Phage - cast her as a spell?" and "did you cast the spell from
your hand, or from somewhere else?".

No. In order to get Phage into play without losing the game, it needs
to have started in your hand, and you need to have played it. Fist of
Suns allow you to play it using an alternative cost - but you still
play it, and it can still be from your hand.
