Flash drive not working

OK this happened twice now. I tried installing Windows on my new PC from a flash drive and it failed so I decided to use the front USB headers on my case and now it doesn't work on any computer. Now I tried using another flash drive and it happend again. I want to know why it did this and if I can fix it.

Perhaps someone wired the front usb ports incorrectly. Without knowing which case and motherboard you have all I can say is the info should be in the respective manuals.

I made the PC. The mobo is an Asrock 870 extreme3 r2.0 and case is Rosewill Galaxy 01-a.
