Fortnite Stuttering Lag (Need Help)

So beforehand I had somewhat a good time running fortnite, but now I have this noticeable stuttering/freezing lag every few seconds or so even when I just like mine a tree
My PC Specs:
OS: Windows 10
CPU: AMD Phenom II X4 945
Ram: 4 GB DDR3
GPU: MSI Geforce GTX 750 TI
My SSD is 198 GB with like 102 GB free
(FPS is not a problem I can very well reach the 60 fps mark and I cap it there but it can reach the hundreds)
I just get stuttering and freezing and it gets really annoying, I know my pc is not up to date nor that strong, but beforehand I had a decent time of running it. Now loading fast enough into spawn island is rare. Also when I play I see in task manager my memory and disk usage is blasted all the way to 100% and I have no idea why.
