Question Frame tear every other second?

Turn on my new pc and have tears every other second.
rx 5700 xt
Have any ideas on what may cause the issue?

Is this in windows or in games?
What monitor are you using?

For games: Vsync On/Off in game is used to stop screen tearing in return for a capped frame rate.

Both , and using Acer Nitro VG272 Xbmiipx 27" Full HD (1920 x 1080) IPS AMD Radeon FreeSync and G-SYNC Compatible Gaming Monitor, 240Hz, Up to 0.1ms Response Time, (1 x Display Port & 2 x HDMI 2.0 Ports)
have tried vsync on and off, either tear.
tried adaptivesync on and off too, wasnt the issue.
