Question Free App or Web Tool To Create Report Based on Answers

Hello all,

I need to create a questionnaire that I will give to several people and with their answers I would like it to generate a report / summary. Some of it would be long form, some of it would be select all that apply boxes, and some of it would be fill in the blanks etc. Can anybody recommend a web tool or Android app that would allow this? Has to be free.
