Game hangs, Screen blacks out without audio!!

Hi,
During gaming, the game just hangs for 2 seconds and then the screen blacks out without audio.

Tried Alt+Tabbing, the display won't come back ON until restarting the PC.

What could be the issue here, it happens randomly and during gaming only.

My Config

CPU:i5 3570K (Hyper 212X)
Motherboard:Intel DH77KC
Ram: 2*4GB DDR3 Corsair Vengeance 1600Mhz
SSD/HDD: 250GB 850 Evo(Boot)/3TB Seagate Barracuda(2+1).
GPU: Sapphire Nitro+ RX570 Oced (1480Mhz C.Clock & 2100 Mhz Mem.Clock) Latest Driver 18.10.1
PSU: Seasonic S12ii-620W
Chassis: Antec P8
OS: Windows 10 Pro 1803

Checked the temps of CPU and GPU, it is well below 55C.

Happens in all games, like Dota 2, ACO, WWE 2K19, etc.

Any possible way to identify that the PSU is the issue here.

I don't have another PSU to check, can i use some software monitoring utilities like HWinfo.
I even have the test results but cannot determine what has caused the issue.
What statistics should i check in this scenario???

I will also try to reduce my overclock to see whether it helps!!!
