[SOLVED] Games freezing/crashing. Why?

Here are my specs
CPU: Ryzen 5 2600
16GB RAM
GPU: RX580
Whenever i played games like assassins creed 4 a couple seconds after i launched the game it freezes. However, i hit alt + esc and then i enter back into the game and the screen unfreezes but freezes again. ALso, there is sound coming through so i know the game is still going on in the background but the screen just refuses to work. Can i get some help please?

So just the game freezes and not the entire PC? If so, it sounds like a driver issue.
Which launcher are you running it from(Steam, UPlay, GoG, etc)?
