Games keep freezing/crashing

hello all,
I recently decided to install and format a second hard drive on my PC for extra storage. However, after doing so I found that any game I have tried to boot up crashes or freezes before I can get into a game. After monitoring task manager I noticed that whenever I opened up any game or application my disk usage spiked up to around 100% and stayed like that for around a minute. As well as this, I noticed that whilst gaming my GPU usage was around 90% and upwards and that my GPU usage suddenly spiked up to 100% usage and back down. I was receiving the usual error 0xcd000012d until just recently I got an error saying I didn't have enough free video memory. I have had this problem for around 3 weeks and have't been able to find a solution. Does anyone know what the problem is?
Specs:
CPU Intel Pentium G4600
Mobo- ASRock B250M HDV
Ram- Corsair vengeance (8x1)
Storage- Seagate caviar blue 1TB HDD (both HDD's)
GPU- GeForce GTX 1050Ti
PSU- Corsair 450M 80+ Bronze
