Go to the western Underdark, the area with the kuo-toa. There you'll
find an altar to Demogorgon. Have a sacrifice step onto the altar (I'd
advise using a summon rather than a party member. Just a thought) and
be prepared for a tough fight.

Ahhh, found the altar, didn't try any sacrifice though.
