Now that I think about it, that's what thermal paste is for. Apparently "Lapping" the CPU is so the small gaps and uneven things are not there.
 
Yes and even with that process you still need the thermal paste because it won't be perfectly smooth.

As for the water cooling remark unless you have an open loop (not the closed ones) it's better to just go for a high end air cooler.
