GPU breaking down

I have ASUS GTX 960 2GB gpu.

So when I play graphically demanding games my computer freezes totally I can't bring up the task manager or anything and I have to reboot it. After rebooting the PC, my graphic card remains turned off, and I have to reboot the PC again after a minute or so for the GPU to turn on again. It's unlikely that I'm pushing my gpu too hard with these games since one of them(witcher 3), which causes most trouble, I played 2 years ago when the computer was brand new without any problems. Once the PC boots there is no error message, not even after the first reboot since the first time it happened I switched to the integrated graphic card and there was no error message on the screen. The last time it crashed tough, I was offered by windows to do the system repair.
I still have the warranty for the entire PC but I wanted to check here first before sending the PC away, if it happens to be some problem with the software.

For the note the crash also happens most often when I just turn on a game, and rarerly, if ever once I play it for some time, so the overheating shouldn't be a problem.

Sorry if I sound dumb, i tried to explain the problem in as few words as possible not to bore you.

Edit: Mass Effect andromeda would sometimes after crashing give me error about how either the directx 11 crashed or the graphics card was physically removed, though this wouldn't crash my entire PC, just the game. Also tonight when I opened my PC case I touched the GPU gently and my pc then crashed, also whenever the pc crashes and graphic card turns off it's only the feed to the screen that get turns off, the lights on the graphics card still glow and ventilator keeps spining.

Update: I just installed windows 10 on my PC, I've played witcher for few minutes without anything going bad then I accidentaly bumped the computer case and the game crashed, and an error popped up about how display driver crashed and it recovered, I've looked it up it says that this could because of a bad PSU.
 
Solution
Could be the power supply, could be a bad card, maybe RAM. Is anything overclocked? Did you check the temps with Hwmonitor or another utility?
You provided no information on your computer besides the fact you have a 960. You should know that when you are not stressing the computer, the temperatures are low AND there is a low demand for power. When you game, the temps may rise, they usually do, AND a greater load is placed on the power supply.

Your issue could be the videocard, or it could be the power supply. A cheap power supply could, over time, become less and less capable of putting out enough power for your system. Sometimes, when you first fire up a game, the videocard and CPU can temporarily draw maximum current as they switch from low power 2d desktop mode to full speed gaming mode. That might explain why it happens when you first fire up a game.

I personally doubt this is a software issue.

PSU:550 w CHIEFTEC A-135 sieries, AOS 550SB, ATX v2.3 140mm vent PFC
Cooling: Cooler arctic cooling freezer

I was monitoring the temperature of the GPU, it didn't go above 80C, that is while playing witcher 3 it mostly stood there at 80 the whole time, it was a bit hard to track it considering that once the computer froze I couldn't see what was the last temperature recorded, but once while the program was loging it the temp was 60C before the crash, though sometimes I did hear the fan starting to work faster once the PC froze, I'll try to download the hwmonitor and see if anything is out of the ordinary.
