GPU Crashes After 4 Minutes of Playing a Game

So I was playing Life is strange and my gpu just crashed, specifically what happend is that my monitor lost connection to the computer, I thought it was a bug so I played the game again and the same thing happend, I was pretty confused so I played another game: rocket league, in the beginning I didn't know what was going on, but now I think I have pin pointed it to my gpu because when I was playing rocket league, 10 seconds before my computer crashed, the frames were terrible, so I believe the GPU is the problem. Also, I tried turning off my monitor and then turning it on, it didn't help. I just had no signal to my computer. I have to restart my computer for it to work.

Also, another important note is that I had to restore my windows 10 because I was having an issue before which was that when my computer was idle, it automatically restarted, so I restored it, downloaded drivers and here I am. The issue of my GPU crashing didn't happen before that. Please help me, I really need advice.

Also, my GPU is a 1.5 GB gtx 760, pc came prebuilt it was a HP Phoenix 810-209: http://support.hp.com/us-en/document/c04337646

I assume you installed win10 directly and did not do the factory restore to win8.1 and then upgrade again to 10...
So did you reinstall all These drivers ?

What I did was I reset my PC and reinstalled win10. so yeah, did not restore to win8.1 and then upgrade to win10. I downloaded all the drivers and it's still not working. Though, I did get the chance to see the temperatures just before my GPU crashed, it was at 102 C.
