GPU drivers crash, black screen, hard locking, or "Critical Process Died" constantly

Login to windows and either my GPU drivers crash (latest and last 980 TI drivers), black screen requiring a reboot, or hard locking requiring a reboot.

What I've done: Replaced every single component of my PC except the graphics card that is 3 months old. Tried reformatting Windows, but I get an error and told "Cannot refresh Windows 10, no files deleted", obviously ran malware tools. I suspect it is either the motherboard, CPU, or PSU; however, those were the original culprits and were replaced today.

EVGA 980 TI SC+

i5 4690k

MSI Z97 Gaming 5

EVGA Nex 750w Bronze (I know, I know. I had to replace my 5 year old PSU when this all started because it was the most likely culprit and that is all I could afford and stretched it a bit)

Windows 10 64-bit

Run system file checker and see how that works out.

If You're bound to re-install Windows, don't fear - If You have the DVD then do exactly like his:
Start windows normally (If You can) don't do anything but start Explorer > then with the Windows DVD in the slot Run Start.
Windows installation process will start.

After a While it will come up with some questions in which you have to answer.
Just do Repair. It will keep all your installed software and while it will replace all of the system files, then it is very important to run Windows update.
Start > Settings > Update & Security => Windows update
Restart
Repeat this procedure until it is no...
I had the same issue with my last card, is this a Factory OC model (yes) try using Afterburner to UNDERclock your gpu to stock speeds.

If it then runs stable, RMA it as not able to run at specified speeds.
 
Using integrated graphics changes nothing (should have mentioned), underclocking also doesn't fix anything, but a new issue popped up. my PC will stop recognizing hard drives unless I reboot.

I'm starting to think my windows may be corrupt and is just having problem that seem like hardware.
