GPU Failing, No Idea Why

When I play games that are graphically intensive, after 10 minutes or so my screen turns to a solid color and I hear the windows error message. I can only hear things but can't see. I looked at the temperatures and they're all fine. I've reinstalled my graphics card's drivers many times and still no success. My GPU kind of slants when it's in place, and I'm not sure if that would hurt the card or could possibly be the reason why it's acting this way. I've attached a picture. Thanks!

Is it properly clipped into the PciE slot? Because it seems too un-straight to correlate with the slot.
