GPU Fans not Running causing crashes

As stated my GPU fans do not start on launch, nor do they start spinning when gpu reaches high temps. I noticed this problem when my computer crashed - after rebooting the gpu was still very hot and I noticed that the fans never turn on. During a test, the gpu got up to 99c before I stopped the test and the fans did not turn on. I have cleaned the dust from my gpu (of which there was very little) and checked the power connection to my motherboard and to gpu. Not sure what else there is to do. Right now my gpu is idle at 80c and Open Hardware Monitor and MSI Afterburner claim fan speed is 100% although it is not. Manual control in either software will not start the fans. I am no expert when it comes to computers, so any help would be appreciated greatly. Thanks in advance!

Solution
Check if the cable that links GPU fans with PCB is properly connected to header, cos it sounds like it is disconnected (or the cable itself got damaged).

While I am not sure exactly what you mean, I have taken apart and reattached every power cord and the graphics card itself multiple times. Other than the actual slot into the motherboard, the gpu has 2 6 pin slots that run to power unit. I was under the assumption that those two ports powered the card and the fans.

So I decided to follow the advice and take apart the gpu. I unscrewed the four spring screws on my gpu and the gpu separated easily. One cable connects the two pieces, and it was disconnected. I figured it had come apart when I took the gpu apart, but it took a good amount of force to reconnect the cable. I am not sure how a cable like that could disconnect itself over time, but after connecting and rebooting my fans began working just fine. I never move my pc, so this problem came as a big surprise. My gpu was initially installed over 3 years ago, and this problem just occurred now. I was not sure what you meant at first DRagor, but I am pretty sure you were spot on. Thanks for the help!
 
It is possible that the plug was not mounted properly at the factory, and fell off during cleaning.
Or, even if you don't move PC at all, working GPU creates so much vibrations on it's own that it could cause the plug to get loose.
Anyway, the symptoms were clearly pointing towards the plug failure.
