GPU or PSU Faulty?

While playing Fallout 4 my screen would suddenly go blank (sometimes it would be grey sometimes green) with white vertical stripes. The computer would become unresponsive and sometimes audio would loop for a while. The only way I could fix it was to manually restart the computer.

This happened a number of times. Then more things started going wrong.
I noticed that if I had my graphics card working and in the computer (with no extra strain, just browsing) my hard drive would not appear. (I have an SSD and a regular HDD) When I pulled out the graphics card and used onboard VGA the hard drive would appear back again. So I worked with onboard VGA for a few days.

I tried putting in the graphics card again so I uninstalled the drivers with DDU and plugged in my graphics card. Now I get these pink/green square artifacts on my screen and when I tried installing the driver the computer restarted. (the HDD did appear though)

I also noticed that one of the fans on the graphics card has stopped spinning.

I am unsure about whether this is a faulty graphics card or a faulty PSU. Thoughts?
