GPU problems - display driver fails to start

Hello,

My build is as follows: Asrock Pro3 R2.0 motherboard, Amd Fx-4300 cpu, Gtx 960 gpu, 111 gb adata sp 550 SSD, 1 tb seagate sd1000dm HDD and 8 gb dual channel ddr3 running at 1600 mhz.

I had problems since the beginning. While playing games and watching videos I could notice delays/stuttering which didn't affect sound but were very annoying. These problems would only be present at the next start after using shutting down option(not after sleep). However I found a fix; if I would restart the pc the stuttering would go away and everything would work perfectly. But not even restart would fix it every time. If everything worked perfectly and I would restart a second time the vid delays/stuttering would be back again. So it works only every second restart.

Recently I decided to upgrade to windows 10 since I've had 7 for quite a while but the problems seem to be even more pronounced in windows 10.
When I start the pc (after shut down, not sleep) I get the "display driver failed to start" notification and the screen goes black every couple of seconds. Sometimes it's so bad I can barely restart it, but once I do everything works perfectly!
Putting pc in sleep mode and waking it seems to work fine but the problem is that the fans keep spinning and it makes a loud noise.

What I tried and didn't work:
1. Reinstalled windows couple of times (only on ssd). Sometimes I would let windows update install the gpu driver, other times I would do it manually from Geforce website.

2. Updated. uninstalled and reinstalled gpu driver from device manager. Running on microsoft basic display adapter seemed ok but I can't do anything with it.

3. Updated UEFI

4. Disabled fast start up from bios and win 10 since I thought that something might not be loading properly but since it does that every second restart too I had to know it wouldn't work.
I'm not sure what to do next.

Any possible fixes you guys might have in mind it would be greatly appreciated !!

1.I would check which PIN connector you have on gpu 6pin or 8 pin? (which psu unit do you have, brand name pls and wattage)
2. I would uninstall drivers with DDU (display driver uninstaller) in safe mode
Open the Start menu and click or tap on the Power button.
Then, while keeping the Shift key pressed, click or tap on Restart.
Then, Windows 10 reboots and asks you to select an option. Choose Troubleshoot.
Then, on the Troubleshoot screen, select Advanced options.
On the Advanced options screen, choose Startup Settings.
Windows 10 says that you can restart your device to change advanced boot options, including enabling Safe Mode. Press Restart.
And do the DDU uninstalling (run as administrator).
3. How did you install drivers for GPU...

3. How did you install drivers for GPU? Automatic or selected your gpu?
4. I would let windows update to its latest point then install drivers normally (as some drivers cannot install itself).
5.Stuttering may be because of CPU voltage drops (as i've experienced in past) and also of your PSU/GPU soo i would download CPU-Z and check first tab and post screenshot of it here.
6. I would go to bios and picture H/W monitor tab (is your bios to latest point)?

Basically "4." worked. I was either installing windows and the lan driver, let it install and update everything, then I would get the stuttering and reinstall gpu driver or installing windows and just download and install gpu first from the geforce website and then the lan driver, since the rest was done by windows update. After reading your post I went to the motherboard support website and even though I got the idea that "AMD all in one" driver is for amd gpus and since mine is from invidia I don't need it, I went ahead and installed it. This fixed everything, no more stuttering, no more black screen no matter if I shut down or restart or put it to sleep!
 
I have been getting this over the last 2 or 3 days in Windows 10 1803 with an GTX 960. About 5 years a go I was getting the same error on a GTX 560TI card. This had been going on for about 3 or 4 years and only happened about once or twice a year. Over time this error kept on happening more often until one day the computer wouldn't recover. I managed to boot up is safe mode and go to the default mb graphics. Got out of safe mode and booted up as normal with the mb default graphics and found that my GTX 560TI was no longer working. Purchased what I'm running now. My conclusions to this problem are either your graphics card is on the way out, graphics card getting to hot ( I found in my Asus GTX 960 software that the fan had been some how turned to quite mode, which is not good for gaming) or you may have a intermittent contact on your PCIe slot. Take graphics card out and it is out give it a clean and then put it back in, just by doing this may just clean the contacts. These are only just my observations.
