Alright, I've literally tried everything here and I'm about to throw the towel.
I've got a i7 - 7700 paired with my good old R9 280 Dual X 3 GB edition.
I'm unaware of how long this issue has been persistent here, but I was playing Witcher 3 and I noticed ridiculously low frame rates. Configuring Afterburner to show my clock speed, I noticed a miserable 470mhz compared to the 940mhz my GPU should be running at.
So I reinstalled drivers and it worked... for 1 day. Restart the PC and we're back to square 1. I tried doing a clean reinstall of drivers, removing any old drivers and installing new ones but the issue is still here, my GPU just REFUSES to go full speed. My temps aren't high and I made sure that they stay under 75.
