GPU Upgrade on my old computer?

I have an old computer (HP Pavilion) and has an Intel Pentium Dual Core CPU E2200 @ 2.20 GHz and I have a Geforce 9800 GT. I want my performance to go up with a new graphics card and want to be able to have something that can run ANY game at atleast medium to high settings. I want a GPU that won't be bottle necked though. I know that it might not get bottle necked but I am new to the PC community and want to know if I could get a good graphics card. I also have a $150 maximum. Thank you in advance!

With today's games, you will need to replace that rig. Some games require quad-core CPUs and at least 4 GB of RAM. For some lighter games, though, such as some Steam indie games, you can get by with an R7-250 or a cheap GTX 650 if you can find one online. Older OEM PCs will not take NVIDIA 700 series GPUs.
