GPU Visual Anomalies

Recently I've been noticing 'flashing pixels' and screen tears that have been occuring not just in my game, but also on my desktop as well. When I try to run a low-end game, it'll happen every so often; sometimes they will go away and reappear at times. However, when I play a high-end game, these visual anomalies appear like this:

It's gotten so bad that I've gotten a blue screen just by trying to play a Facebook video

Here are my specs:
Motherboard: MSI FM2-A78MA-E45
CPU: AMD Athlon X4 750K Trinity @ 3.4 GHz
GPU: NVIDIA GeForce GTX 760 (EVGA) 2GB
Storage: 1 TB Hitachi & 240 GB SSD Kingston
Memory: 16GB DDR3 (4x4) Ripjaws X @ Hyper X
PSU: CORSAIR RM @ 850 Watts (80+ Gold)


I've updated my drivers (as well as uninstalled with CCleaner and reinstall several times):

I've cleaned this GPU thoroughly twice (removing the 4 head screws and getting dust out)

Is my GPU dying, or is it something else? I don't see any screen tears when I'm just browsing, but when I open certain games, it's just red and blue flashing pixels everywhere, with occasional screen tears. This also happens on YouTube, but it happens time to time.
 
Solution
It's almost definitely a defective video card. More specifically it's the VIDEO MEMORY.

If you go through EVGA technical support I'm sure they'll tell you to RMA the card for a new one.
