Question Grainy and pixelated display output ?

Hi everyone! I've been having pixelated and grainy display output for months on end now and cannot seem to find a solution.

Specs:
GPU: EVGA 3070 FTW3 Ultra
CPU: Ryzen 7 5800X
Mobo: ASUS Tuf Gaming X570 Plus (Wifi)
RAM: Corsair Vengeance 32gbs @ 3200mhz
PSU: 650 Watt CoolerMaster Gold Cert. Fully Modular
SSD: 500gb Samsung 870 EVO
Monitor: Dell S2522HG

Solutions I have attempted:

1. Replacing hardware (GPU, CPU, Motherboard, and Monitor are all brand new)
2. Changing various in-game, Windows, and Nvidia settings.
3. Uninstalling drivers with DDU COUNTLESS times. Reinstalling from both EVGA and Nvidia.
4. Clean installation of Windows 11.
5. Testing on different monitors, one ASUS and two Dell monitors. Exchanged cords and even used each HDMI and DP output and input.
6. Uninstalling and reinstalling games.
7. Praying to the PC gods above to magically remove the issue.
