Graphics card broken, or something else?

So my problem starts with me playing h1z1 king of the kill.
i installed the game last night, but i wasnt able to play it since it was kind of late, so i just installed it and turned off the pc. next day i tried to play it, started a match, lost it, but when i tried to exit the match the screen just froze, and seconds later some blue horizontal lines appeared on top. And few seconds later the monitor turned off showing the 'no signal' text.

I restarted the pc and i got the same problem, but now there was a black screen with the blue lines on top. I dont event know what caused this.

I tried reinstalling the windows. (windows 8.1 pro). i was able to get back to the desktop but it was like the gpu wasnt detected. when i opened device manager there, instead of my gpu, it was shown 'Microsoft Basic Display Adaptop'.

After seeing that, i tried to install the gpu driver, but during 'Graphics Installation' the display just turned off for 1-2 sec and when it turned on again, it got the same blue lines on top of a black screen.

I tried to search the problem on the internet but i failed in founding anything like this.
Is the gpu broken? or is there a problem of windows or something else?

i will list my pc specs down here
motherboard: msi h110m pro-d
cpu: i5 7500 3.4ghz
gpu: asus gtx 1060 6gb strix
ssd: kingston uv400
psu: Sirtec - High Power EP-500BR II, 80+ Bronze, 500W
hdd: 1tb
ram: hyperx fury 8gb 2133mhz

1.Uninstall the drivers USING THIS PROGRAM not via windows uninstaler!!! : http://www.guru3d.com/files-details/display-driver-uninstaller-download.html

2.Install the newest drivers from nvidia website : http://www.nvidia.com/content/DriverDownload-March2009/confirmation.php?url=/Windows/382.33/382.33-desktop-win10-64bit-international-whql.exe&lang=us&type=GeForce

3.Make sure you are using windows 10. It's recommended to use it.

4.If you want to use your 8.1 pro , install the new drivers after you uninstall your current one using the program I told you to use.

If nothing helps out of these steps I will help you further. MAKE SURE TO USE MY PROGRAM! If you don't use it you are not whiping out all driver related files and it will not work. Hope I helped.

Does it look like on the picture all the time after you installed drivers after Win reinstall? If yes, it's pretty much dead GPU. You can test it at another machine to confirm.

He did a full reinstallation of Windows which makes your suggestion out of place.

@ Chrrisstian_1 I too believe it's your GPU. If it's possible, try to RMA it.

Yes I did. It was my mistake that I wrote that. Sometimes it helpes to do it twice. It was like that in my case with gtx 950.

if my gpu is dead, do you have any idea why? regarding how i described the situation.

In my opinion there is something going on with the core then. I had simmilar experience with my gtx 660ti , so I wanted you to give it a try with the steps I gave you. If none of this worked the only thing that is left is to RMA the card.

You should take out your card and put it back in. If that don't work your gpu could be damaged or it's a motherboard issue but that is very unlikely.
