Question Graphics card fans running at max speed but software showing 0% speed

I've got a PNY 980ti GPU that I've had for about 5 years now.
When I first got it I flashed the bios so that I could have the fans stop running so they could be in a 0% rpm mode, like the Strix models of GPU did.

However, after about 2 years or so of having the card and having had it flashed, the fans would randomly lock themselves at 100% speed.
It very rarely happened and would usually be solved by restarting the computer.

about a year after that I moved to Japan for a couple of years and let my dad use the card, he said it began to happen more frequently and eventually went back to his old one out of annoyance.
And now I've moved back home and am using the computer again and it does it every time as soon as the computer is powered on, not even waiting for windows to boot.

Does anyone have any ideas on what might be the cause? I don't think its software related as its been tried on 2 different computers and starts max speed at power on.

tl;dr GPU runs fans at max speed constantly but software shows 0% speed.

Any suggestions for a cause would be greatly appreciated.
