Hello there people!

Lets get to the point without wasting time, I am using Nvidia GForce 5500 ((256 MB)) but fews days ago it started freezing the screen and when I restarted my PC it started to show some somes lines on windows's Xp logo and after that the same problem appeared again(( freezing screen)), I have heated too much after this problem and also changed its thermal paste. After changing its thermal paste the lines from window's are disappered and everthing goes fine but now everything goes fine till Xp logo but the as the welcome note come the screen becomes 640x 720x with strange graphics on it and it remain same nor "dxdiag" detect my card nor I can install new drivers and yeah it shows question mark "?" on video graphics in device manger.
When I take out card from my PC everything goes fine and works properly.

Use an airbrush. I think that the GPU has just had it and is dying now. Your best bet is a new GPU like the GTX 750, a new OS like Windows 8.1.
Means when I touch it, its temperature is a bit high and then the screen starts blinking and the screen goes black and when I restart my PC it shows small vertical lines on windows Xp logo and again screen goes black.

Maybe you shouldn't touch it. Most GPUs are designed for up to 80-90 C, so you would burn yourself if you tried touching it. For the vertical lines, it could be a dying GPU. That GPU is very old (one of the oldest GeForce cards) so it should be pretty much end-of-life now. I would say you should upgrade to a new card like a Geforce GTX 750 or a Radeon R7 250X. As for Windows XP and the 640x480 monitor, get a new one that is at least 1400 x 900 or 1920 x 1080. Get Windows 8.1, and it should all be fixed.

One more few months ago, my GPU showed same problem after that I cleaned it with Patrol and heated hair drier, after applying these methods multiple times it started working and I used it for like 6 months and everything was fine and fit.

Strange way to clean a GPU. I would try to get a new GPU and see how that works.
