Graphics Card model

On my notebook I am not sure what graphics card do I have as 3rd party programs check it via the driver but my driver shows only that i have the amd mobility hd 5000's series.
How can I check the exact model?

Use cpu z

I would tend to use GPU-Z, not CPU-Z.
