I have a gigabyte windfore 8gb that has been working well and have had this issue once before but forgot how to fix it. It crashed during a game of LOL and was fine upon restart, several minutes later when the computer went to sleep it had many small green horizontal lines and only showing on one fo my two displays.
I have installed new drivers in every way possible, restarted the computer and let power dissipate, look through my bios and didn't see anything.
It is now no longer showing up in my device manager. I also have a second GPU, an NVIDIA quadro 6000 that i use for engineering.

MSI x399 gaming pro carbon
32 gb RipJaw Ram @ 1465Mhz
