graphics glitch when returning from sleep

Hi everyone
I have a weird issue when waking up my PC from sleep
Almost always it wakes up with the display looking like this

some times it goes away after a couple of seconds and sometimes I need to shutdown and turn on the PC (restarting doesn't works)
Any idea of what could it be?
my specs:
i7-6700k(no OC) and Maximus VIII hero
GTX 980ti(no OC)
Seasonic x-850
LG 24gm77 monitor (connected through DVI-D)

I have the latest bios, and did a clean reinstall of the graphics driver (using DDU)

Could be the drivers. Why is it upside down

Ive had the issue since the drivers that came with the GPU
The picture is upside down because I accidentally took it that way (I just noticed it)
If I totally disable sleep, it has no issues whatsoever, is specifically when resuming from sleep

perhaps it needs some caffeine? seriously though, not a clue. did you check the cabling, just in case?
has the same thing been happening with hibernation mode as well?

I switched to using display port instead of dvi and never had the issue again.
To this day i don't know what was causing it, but i was also experiencing random reboots and freezing issues which went away by cleaning the gpu contacts so it might have been because of a poorly seated gpu or dirty contacts.
