So how the issue goes is as follows. I will be playing a game and then randomly my screen will turn grey/black/green, the audio will switch and play through my television even though I am wearing a headset.
It will then open up another instance of my television audio device which makes the one I was previously on, not work, when this issue happens the game cannot be tabbed back into, it will 90% of the time crash whichever game it happens on.
I've tried a new HDMI cable but it did nothing.

The games it crashes on is seemingly random. The ones I remember it happening on are;
Gears of War Remake for Windows 10
Rocket League
Resident Evil 2 Remake
Risk of Rain 2
Yet I can play a game such as Sekiro and it be completely fine with zero issues. As you can with the dates of some of these games that it's been happening for years.
I've searched quite often when it happens but nothing ever turns up.
My specs are;
Mobo: Gigabyte GA-Z87-DS3H
OS: Windows 10 64 bit
CPU: i5 4670k OC to 4.3
Ram: 16gb
GPU: GTX 980
power supply: cx600
