Okay, so at some point, idk when, after a certain period of time, for no seeming reason, be it after twenty minutes or an hour, I'll be playing Fallout 4 and the screen will suddenly turn grey/white(kind of a mix, light grey I guess), the monitor will then lose signal, and the rig will still be running as if the screen didn't just do what it did. Sound sometimes still plays, but will not change and overall won't respond to input from either the keyboard or mouse, forcing me to make a hard restart. The kicker is it almost exclusively happens with Fallout 4. I can play World of Tanks for a couple hours without and issue, and even Fallout New Vegas and Skyrim, whereas Fallout 4 freaks out after twenty minutes. I've even modified the graphics to be not as high and I get the same result. Updated and even rolled back drivers with no change.
idle temps for the CPU are around 30ish, and get to about 50-60 when running FO4
idle for the GTX 970 is around 40, but gets around 67ish running.

Mm, and on rare cases, the system will completely restart on its own, though that doesn't happen nearly as often

Solution
This is a known problem with the last couple of nvidia driver updates. You need to do a very thorough driver clean, preferably with ddu and go back to the 362.00 drivers or earlier. There is a lot of complaints about this on the nvidia forums.
Does this happen only for games, or is it for anything else? You might want to try and run a low-powered program for a couple hours (Chrome, and nothing else running) and see what happens. Also, to see if it is the game's fault, run a stress test for an hour or so, and see how your computer reacts. Basically, be prepared for a few hours of troubleshooting.

Only for games, Fallout 4 considerably more than any other. Ran a test using Furmark and it got to about 72c before doing the screen thing.

Hmm how unusual. Have you seen anyone else on the internet report the same problem? Because if not, it is specific to you, which makes the problem much harder to solve. You could try and contact Bethesda and explain the problem you are having. Also, although this probably isn't the case, make sure that ALL possible drivers are updated, everything from GPU to monitor to exclude all other possibilities.

There is a bunch of software out there which can help you do this clean, easily and without any errors. Unfortunately, I haven't ever needed to use this software, so cannot recommend any. But a simple search should do the trick.
