GTA5 annoying mission fails

While GTA5 is kind of hit and miss with it's fun factor, a lot of the main story missions are actually very entertaining. I find some of them just have their down right annoying moments. Sometimes it's getting stuck on a piece of scenery when you're on a time limit. Sometimes it's the engine in your car inexplicably dying.

What are the most annoying mission fails for you?

Smashing into a car, flipping off a motorcycle into an overpass, and dying. Mission failed!

In San Andreas years ago, a small plane randomly fell out of the sky and crashed into my car. That was so amazing I wasn't even mad.
