GTX 560 - 3 monitor setup & win10

I have a Geforce GTX 560 card with 2 x DVI, HDMI and Display Port and I'm using windows 10.
Any combination of DVI & HDMI using "Two" monitors will display without a problem.
The problem is How Can I get the "3rd" monitor to display in
windows 10?
When using BOTH DVI with HDMI, the windows 10 "Customize Your Display" identifies the monitors as 1 & 3 only.

with lower end gpu that have only two dvi or one hdmi port you have to use the vga port on the card or the onboard video video port of the mb. most lower end gpu will do two digital ports and one analog. some amd gpu can do more then two. post up the vendor and model of the card and we can do some back checking for you.
