GTX 560 Ti Black Screen After windows loading | BOSD (in windows 7)

Hi everyone
first of all this is my rig
CPU : intel i5 2320 3 GHz
GPU : GTX 560 Ti Twin Frozr ii
RAM : 12 GB DDR3
Motherboard : H61M-VG3
OS : Windows 8.1 64 bits


Recently I've been experiencing some driver error messages saying : nvidia driver has stopped and recovered i usually ignore this error because ill just have to re open any crashed software and everything work fine again , yesterday i was playing Warface ( for hours ) with the highest settings , i was also using MSI Afterburner to adjust a custom fan speed setting

The game crashed i thought its that nvidia error again but i saw no message , anyway i ignore it , later for some reason i rebooted my pc , the windows was booting , after the loading screen the monitor became black and the led of the monitor start blinking indicating that there is no signal .
when i tried to see if the pc working and its just the screen i press the caps lock button ( its a trick i usually do to see if the pc still responding or not ) so when i press it nothing happened wich means the pc is not responding
of course i rebooted more times and still the same ....

Here is the things i tried :
1-Removing the card and totally clean it and clean its slot on the motherboard (no result)
2-Trying login with disabling driver signature( no result)
3-Using a special cleaning liquid for electronic cards to clean the card (no result )
4-Accessing Safe Mode ( i was able to access it)
5-Un-installing the driver totally with DDU and install a LOT of older versions ( no result )
6-Installing a new OS (windows 7) everything works fine till the second i install the driver ( blue screen after the rebooting that the driver installation require)
7-Installing another OS ( Mac OS X El Capitan ) ( gives black screen after loading)
8-Unplugging all useless drives and hard drives to ensure the power supply is enough for the card ( no result)
9-Removing one of the Ram and even tried another new one ( no result )
10-Removing the card and try it on another pc ( same result on mine : BSOD )
11-Tried a different cable and different DVI out slot and even VGA slot on the monitor (no result)
12-Tried another Monitor(no result)

i think this is all what i tried , maybe i forget something but ill add it if i did .

PS : the card wasn't overclocked while playing or even before .
the BSOD indicate error code 116


In my opinion iam afraid that my GPU died but i saw a lot of peoples having the same issue and then they find a solutions but i cant find one , and its not logical to me that it can show any picture if its dead i mean in my opinion i think when a GPU died it should not even show the BIOS ,so please enlighten me with any ideas or things i should try more , and advice on what i should do and thank you :)
