[GTX 680] PC freezes when playing any games.

Chronology :
So, my pc start to freeze when playing games (dota 2 and cs:go). this is not the first time it happen. back in December 2014, my pc fail to start, bring it to shop and got a new psu. issue solved.
then few weeks ago, suddenly the issue started.

Issue :
- 2 weeks ago : When playing games (Dota 2 and cs:go) after awhile around 30min screen freeze need hard reset
After several attempts to fix it
- Today : Screen Freezes for few seconds ( 30~ish ) then back desktop. Games still running in taskbar. but cant seems to bring it to the main desktop. need to end task from task manager. the issues is consistent.

Attempts :
- Tried to fix issue from the software, increased TDRdelay to 50.
didnt solve it, it just took longer to freeze.
- Tried Driver from the cd that come with the gpu,
List of version that i tried : 301,314,352 , didnt change anything
- Tried my cousin GPU GTX 660 , issue still the same. ( which mean its not my GPU ? )
- Tried 1 memory stick to check the RAM stick, even with 1 RAM stick the issue still persist
- Run memtest86 - No error found
- Downclock using MSI afterburner ( -100 ) issue persist.
- Tried Furmark
Default setting - Freeze then crash at 74 degree ( it always crash when it reach 74 ) around 48sec
Tried to increase the fan speed to lower down then degree and make sure it last longer, Still crash even faster. around 36sec.
- Updated Bios , Still same issue
- Cleaned from dust ! Still Same sadly


Side note:
GPU temp - 35 ~ 60+ ( when running the game )
CPU temp - 35 ~ 40
Whenever i run DDU, there some error before restarting and entering safe mode ( not sure what it does )

Spec :
Proc : Intel Core i7 3770K 3.4GHz ivy bridge
Mobo : Asrock Extreme 4
RAM : Vengeance® — 4GB Single Module DDR3 Memory Kit (CMZ4GX3M1A1600C9) x 4 units
PSU : Seasonic M12 II 620W
Cooler : corsair H60
SSD : OCZ Vertex 4 128GB
HDD : WD Caviar Blue 1TB
ODD : Lite-On 22x
GPU : Palit GTX 680 Jetstream 4gb
CASE : corsair 600T

Probably Either CPU / Mobo or PSU ?
Cant really tried the PSU cause my cousin PSU is 550w and some shady brand. also the her only 6PIN
and my PSU not even 1 year old ( pc 4 years old ) since i changed my PSU back in december, changed the same brand and model (Seasonic M12 II 620W)
CPU and Mobo, i will try to find way to check this. since im located in somewhere, that hard to find a decent Mobo and CPU to try. bought the PC when im studying overseas,

Thanks for reading my long frustrating journey, Any help would be decent.
 
BUMP need help. apparently the computer shop cant help me as they dont have the spare parts to test.

New update :
Not sure if its important. when i run furmark. whenever the GPU power reach 100+ ( such as 109 )
it directly freeze. does this mean the PSU cant supply enough power ? or the GPU broken ?
